HOW TO SUBMIT YOUR PAPERWORK

Send ALL documents at one time together to expedite your process. Also, send an original Birth Certificate, a passport photo (Walgreens or CVS can provide), a red, wet ink thumbprint on a plain white piece of paper along with a CLEAR signature. You will need to purchase a red felt ink pad as you will be using this in the future for other documents. SEND DOCUMENTS WITH A LARGE, SELF-ADDRESSED AND POSTAGE PAID RETURN ENVELOPE SO THEY CAN BE SAFELY RETURNED TO YOU FOR YOUR RECORDS AND SAFE-KEEPING.
